- The employer can choose to complete individual applications for employees.
- The company application form and membership template, which are available on Complete Health, are required to process the individual applications.

- If the group has 50 or more employees, an account executive (AE) and internal service consultant (ISC) will be appointed to assist you in servicing the group.
- If you have multiple employer groups, please take note of the allocated AE and ISC, as these may differ per employer group.

- If the group comprises 50 or more employees, the group may qualify for discounted premiums, based on the risk profile and demographics of the employer group.
